As expected, the fan problems have returned with my 3rd Latitude:

I was hoping this one would be different, especially after returning my last two 6400's due to the lack of proper heat dissipation which began to degrade my hardware.
When I received this one two months ago, I was hopeful as I noticed a difference in overall temp and fan usage.
My fear was that after time, the fan would begin to kick in more often due to a rise in temps and/or increased usage (I have the nVidia 160).

Unfortunately that time has come and the laptop is increasingly having a more difficult time to get the temps under control. The slightest work load or, video usage kicks the fan in at top speed.

Not sure what to do now...

Unfortunately this is a problem I have noticed with my previous Dell machines - their temperature management is not that great. It could just be my personal experience, but for example the model I currently have (admittedly the budget Inspiron model) doesn't turn the fan on at all until it absolutely has to, and then turns it on at full blast until it's a lot cooler. I'd much rather it was on constantly at a lower settings, and I am convinced this would be better for the notebook.

There are options available with regards to cooling laptops, I would recommend reading these threads:

Ultimately though the notebook is still very new and it might be worth discussing the issue with Dell themselves.

trust me, I wish more than anything that I had not ordered this cursed nVidia card.
However, no matter what I do or have tried, Dell will simply not change it for the Intel card, even though its considered a downgrade.
I have been through the ringer with them over the past 6-7 months and they will not change anything with my setup, they will only send me an new model with the exact same specs. This obviously gets me nowhere and just prolongs the cycle.
They wont give me my money back either.

Hope you'll get more joy out of that than you've had thus far! Make sure to include the fact that you'd be willing to downgrade your GPU, but I'd add the stipulation that it would need to be a new machine perhaps.

    Good luck.

Perhaps a refund is the best route.

Then buy a version with Intel graphics.
