Has anyone swapped out the MXM card in their M4600 with the M4000? I know they are both MXM 3-A slots so that should be fine, but any reason why it wont be recognized by the system?

It will work fine as I've tried the same upgrade with my M4600. There are a few dirty cheap Dell M4000 cards right now on Ebay . Make sure that you buy the Dell card, as the HP ones might not be recognised. Also you could wait a bit more and upgrade to firepro M5100 or K2100M. There is no reason to believe they won't work either.

Does AMD Switchable Graphics work in this combination Dell M4600 with AMD M4000?

No, M4600 supports switchable graphics with NVIDIA cards only.
