Files that have long since been deleted show up when I use search. I have indexing turned on so the file shouldn't be showing up anymore. Is there some way to get Vista to remove old file entries or refresh its index?

Yes, you can rebuild the index.

Click on the Start Orb, type "Index", and click on Indexing Options. Then, click on Advanced, and then click on Rebuild.
