Roxio software (but it costs money). I made a perfect image of an Office Enterprise 07 disc (from the Partner Program) with it.

No, but I have had good success making and burning images with BurnAwareFree (it's freeware). Have made at least 40 each images and burns, with nary a failure.

I have a copy of Nero 8, and from what I have seen, it's the same thing, just not as fancy. FWIW...

CD2ISO is a tiny .ISO ripper. Nice and simple. Works for me.

No. DVDshrink is for ripping dvd videos.

Alchohal is a free cd/dvd burner or imager which is also free

Regional settings has nothing to do with DVD image. DVD image has no regional settings.

Or.. you're not creating an image at all. You're just ripping the dvd video.

I did all of my stuff for my httpc with dvdfab5 it was the only ripper I could find that circumvented all of the various copy protections. Many good programs out there but they only worked on about half my dvds.

I ripped to .vob though and then encoded it to a .mp4 with h264. same quality smaller size. good for htpc but not what you would want for a "copy" of the image. For that I do not recall if dvdfab5 does an .img rip.

You may be best off just doing a 1:1 copy from one disk to another. I personally would not store original dvd images on my hdd just seems too waistful.

It did atleast. When I ripped the dvd there was an option to remove region settings and after that the movie played but before that, the movie came green, slow and glitchy while playing with just a dvd image.
