I just wanted to clarify something, do I need to run it every time windows starts for my undervolt to take effect.
Also do you just adjust the core voltage or is there anything else you should change too.

You can alternatively use startup scripting and the command line part of XTU to run your settings automatically without needing to open the XTU program (I have to do this on one of my older systems that sets low turbo power limits on every boot)
What settings in XTU are available depends on what is locked out depending on CPU and what is locked out by laptop maker. If HQ or U, multipliers can't be raised, power limits are fixed, undervolt may not even be available; on unlocked platforms you may be able to raise turbo limits and times to get the CPU using its max possible multi at all times
If undervolt is all you want to do consider using Throttlestop instead, its much more feature rich and less resource intensive, without background services
