So I go to disk utility and run the permissions check, it comes up with a bunch of permission errors, or rather "permission differ" so I fix this and re run the scan, and again the permissions differ. I then try doing a clean install of lion and running disk utility in the boot menu, and I still get this.. Should I worry about it at all? The computer seems to be running ok..

I don't know how disk utility is supposed to work, but i can't imagine that you should get any errors if everything checks out, therefor i would suspect hard drive failure.

Why are you doing a scan again if you are reinstalling lion again? I would reinstall lion and then just import the applications (not the SETTINGS) through migration assistant. My fan used to run on high, and it was because I imported everything from my older macbook when they used to burn while running.
