I am currently idling at 50C to 55C and when I game the max it will ever get to is 88C.
If i undervolt the card will it make a huge difference in the temps??
and if undervolting is the way to go is this the guide i should follow??

well im on 1.05v on my 6990m and my memIO temp is idle at 46-50c thats on 100/150 (make sure thats your idle clocks),and on 100% load it doesnt go past 90c. Now your idle does seem a bit high, but your max load temps are good. Since your temp problem seem to be at idle, changing your 3D voltage wont do your any good, u would have to lower your idle clock volts which is set at 0.9 which is also the same on a 5850m. i believe your problem is something else maybe a repaste or more/fresh padding, but again make sure your idle clock are 100/150.
PS i did undervolt with rbe and winflash fyi -
If i decide to lower the idle clock volts whats a good number to set it at?? -
I've never lowered the 2d clocks voltage so i cant give u any pointers, your pretty much on your own. lol, good luck

the only advice is dun underclock but just undervolt it also not too much
my frd tried to underclock+volt it and it cant turn on,then he just blind flash it back so make sure u dun do anything stupid
besides why u care idle so much?my 5850m is also at 55c when i just browsing forums
