Did you use any sort of HD texture pack? Because some texture pack like LB Photo Realism can drain your fps down, and also did have install the Java 64 Bit? And finally have you add the minecraft.exe to NVIDIA Control Panel? Btw this is how my video setting look like:

I'm using 16 bit textures, on 64 bit, and using the .jar launcher, also playing on 1080p monitor, 4gb ram

You might want to read this thread, it show you how to allocating more memory to Java hence increasing the framerate, for me I use around 2GB for Java, and my average (maximize) frame was about 100 ~ 120 (I'm using the LB Photo Realism 64x64 texture).

Whitelist the java.exe (or was it javaw.exe? I forgot) in
    C:\Program Files (x86)\Java\jre6\bin\

To be safe whitelist both of them, but for me I only whitelist the minecraft.exe and never had any problem with it though.

He may not be running it on his GPU, my m14 did that for some time at first.

Right click and select "run with high end Nvidia processor" Or something like that.
Should solve the problem. If not get back to me.

If you going to right click and select "run with high end Nvidia processor" anyway, then why not whitelist the .exe to NVIDIA Control Panel because that's a permanent solution, for the Right click thing it will reset every time you start the game.

because that would make too much sense and we cant have that

joking aside it could be a driver update that caused the slowdown if you still use the nvidia card

I had the same issue but white listing fixed it. I think I had to whitelist both minecraft executable and 64bit java, but I have it set up to run at 64-bit java so that could be why.
