I have an Alienware 15r3 connected to an Acer XB241H display via mDP to DP cable. Everything works just like it should, both my internal and external display are gsync, the external runs at 144hz and the internal at 60hz.
Now the strange part is, some games like Witcher 3 and Vermintide for example experience rather strange lag when in menus while using the external display. They work 100% fine without using the monitor. I have absolutely no idea what could be causing this, its also not just on games, but certain applications cause the system to lock up and stutter as well such as AlienFX for example. When switching to HDMI, it works fine again, but obviously I can no longer use the 144hz refresh of the monitor.
Everything also works fine when streaming to my Shield TV, it only happens when using the external monitor.
Any ideas on how to resolve this issue would be appreciated.

Revert you Nvidia display driver to 378.91 (Notebook edition). This solves the G-sync stutter problem. Make sure you use DDU (In safe-mode) to remove your old Nvidia drivers first and make sure you check "perform a clean installation" when installing the new 378.91 driver. I hope this helps.

That did the trick, at least until Windows decided to update to outdated 382.05 overnight. I even used the "show/hide windows update tool" to try and prevent that from happening. Guess I'll have to wait until either Nvidia or Microsoft gets off their butts and fixes this issue, from what I've gathered it's been going on for months now.

You can disable NVidia driver updates. click on the link below and read from "How do I configure NVIDIA Update?". This worked for me bud.

The update option in the Nvidia control panel isn't there for me. I disabled it in GFE and used the Show/Hide Windows 10 Updates app again. Hopefully it will stick this time.Stress Tech likes this.
